Download the FPSC Bio Data  Form . Follow the following steps to fill Bio Data Form .

1. Download Bio Data Form from the Email received from FPSC or Click Here to Download.
2. On the first page you will provide Consolidated Advertisement Number, Case Number, Closing Date, Post applied for , name of the Department , Your name , All Academic Qualifications, Courses Trainings and Research Publications.
3. On the second Page you need to enter Jobs Experience one by one in ascending order and provide all details.
4. On the third page you will need to paste your picture and fill the checklist of all required documents whether you have attached all the documents such as CNIC, Latest photograph mentioning your name and father name on its back side , Copy of matriculation school certificate (proof of age) , copy of all required degrees, Proof of Post qualification experience , Copy of Self Domicile Certificate, Original Departmental Permission Certificate (Only for Government Servants) and Disability Certificate and other document such as driving license for motorway police patrol officer if  required etc.
5. At the end must put your signature on page 3 after the checklist , there are chances your application might be rejected if you will not put your signature .
